Navigating Hospitalization & Home Transitions for Dementia Caregivers

Equip yourself to navigate hospital visits, advocate for your loved one, and confidently transition back home as a dementia caregiver.

Caring for a loved one with dementia presents unique challenges, and few situations are more stressful than a hospital visit. This workshop equips dementia caregivers with practical tools and confident strategies for navigating the hospital system from admission to discharge. Participants will learn how to communicate their loved one's needs to medical teams, recognize and prevent dangerous complications like delirium and sundowning, and manage anxiety and distress in real time. We will also explore the critical transition back home and connect caregivers to community resources that support long-term wellness for both their loved ones and themselves.

Key Points Covered:
  • Understanding the hospital environment — How hospitalization uniquely affects people with dementia, including the risks of delirium, sundowning, and sensory overload, and what caregivers can do to reduce these risks
  • Building your hospital toolkit — How to prepare a personalized care summary, advance directives, and communication guide so you are ready before a crisis strikes
  • Advocating during the stay — How to communicate effectively with doctors, nurses, and discharge planners; know your rights; prevent common complications; and ensure your loved one's dignity and comfort are prioritized
  • Managing stress, anxiety, and redirection — Practical techniques for calming a distressed loved one, using social and cognitive engagement as medicine, and mastering the art of redirection
  • Bridging to wellness — How to navigate discharge planning, coordinate follow-up care, and access community resources that support recovery and caregiver well-being in the critical weeks after hospitalization


This workshop is being offered by Kimberly Reckert, Marketing and Public Relations Strategy Manager for Professional Care Match. She holds a B.S. in Psychology and brings 12 years of community health improvement management experience within complex healthcare systems, with a focus on addressing social determinants of health among vulnerable populations, including chronic disease prevention, access to healthy foods, and nutrition interventions. Kimberly’s experience includes working with multidisciplinary teams to develop innovative programs, including a prescription/food pharmacy pilot for cancer patients and community/clinical linkage interventions targeting high infant mortality rates among at-risk populations. She served as an active member of Food Policy Councils and the Massachusetts Food is Medicine statewide planning and advocacy Steering Committee, regularly providing policy and legislative testimony at local, state, and federal levels. Additionally, she has managed training programs and supervised Community Health Workers in home-visiting interventions that address root causes and environmental factors impacting health for patients and their families.
